ED and SMC-K: A 0% Rating That Pays
Few veterans want to raise it, and that silence costs them. When erectile dysfunction is service-connected, the VA adds Special Monthly Compensation (K) of $139.87 a month, and it stacks on top of any rating, 100% included.

How SMC-K Works
The rating schedule lists ED under DC 7522 at 0%, with a note to consider special monthly compensation. Because it counts as loss of use of a creative organ, it qualifies for SMC-K:
- $139.87 per month in 2026, roughly $1,678.44 over a full year
- Paid on top of your regular monthly compensation, whatever your rating, up to and including 100%
- Leaves your combined percentage untouched, so it won’t move you into a different Illinois property tax tier

The Usual Culprits
Prescriptions
SSRIs and other drugs for PTSD, depression or anxiety top the list, along with certain blood pressure medications. If a service-connected condition put you on the pill, the side effect can be connected too.
Underlying conditions
Type 2 diabetes and prostate cancer treatment, both presumptive for Agent Orange exposure, are frequent causes. So are back conditions that press on nerves and traumatic brain injuries.
In most of these claims, the deciding document is a medical opinion that names the specific cause.
Women Veterans Have the Same Path
Female sexual arousal disorder is rated 0% under DC 7632 and also reviewed for SMC-K. A woman veteran whose condition traces to service, to a service-connected condition, or to medication for one can receive the identical monthly amount.
Erectile Dysfunction & SMC-K: Common Questions
Why file for something rated 0%?
Because the 0% rating is what opens SMC-K, $139.87 a month in 2026, paid on top of your regular compensation.
I’m already at 100%. Can I still get SMC-K?
Yes. SMC-K is added to compensation at every rating level, including 100%.
What does the VA need to see?
A current diagnosis and a medical opinion tying the ED to service, to a service-connected condition, or to medication taken for one.
Is SMC-K paid more than once?
It is paid per qualifying loss of use, and the law caps how many SMC-K awards can be combined.
